Teams
Put several specialists on the same problem so the work gets checked by someone whose job is to find what the author missed.
Every team ends with a fact check
The Fact Checker sees the finished work last — after the lead has revised, because the revision is exactly where a plausible number gets added to satisfy a critic. It is never allowed to check a claim against the work that made it, and it cannot be taken off a team.
A team never widens what its members may do. Each specialist works inside the same job description and the same autonomy settings as when it works alone.
